You need to perform migration. Go to Components -> Community Surveys -> Migrate -

If you are looking only for blog pages, just create Blog Category menu items for the top level menu items. -

From the urls what I can found is there are no menu items created for quizzes to map to a particular url. Always create CQ Category menu item for TOP level... -
