You are correct. The email is sent second time because the new question workflow is being triggered after the approval. Will fix this in next revision. -

You might have trashed them but not permanently deleted. You need to filter by trashed status and delete them permanently. -

Yeah, I see now that it's getting over-ridden (locked) permissions. Stupid Joomla ACL... How to over-ride these stupid LOCKS?? :) com_cjforum.category does...
