I went to the above URL, click on Test menu and List menu. Both loads the surveys fine. Did you already fixed the issue? -

I also have this error with the GPX profile and 'b.split is not a function'. I have applied your fix to line 38 using 49 instead of current. But I still get the error message.... -

No. The system plugin causes issues by caching the pages and they do not render properly. Better not to enable it. -

I did not use the slider before the multiple questions was a bit unclear. But then it seems to be something else because in my production site i did not have a slider...
