There is no menu available for it. However, you can access Most Answered Questions and Unanswered Questions from the toolbar menu on front-end. -

What is the URL in the email sent to you? The URL in the editor may not be the actual URL. Send a test mail and see what is the result. -

That looks to be a bug. The update can be done from your Joomla updates page (Extensions -> Manage -> Update) or you can manually download and install the package. -

So yes, this plugin was deactivated. And I still end up randomly with only the public categories visible on the list of categories. Many of my users have the same issue.
