I could not reproduce this issue. I guess you have some layout overrides in your template, please check and remove them. Here is the result of the demo site page. -

Hi Maverick, just to let you know, as a last resort I reverted back to 4.9 and the survey page now works. I will wait to hear from you before I upgrade... -

The problems that you mentioned are related to your template, how are they solved by fixing GPS Tools? What exactly the fix needs be?
