Check if no other extension is conflicting with pdf generation. As far as I remember plugins from Fabrik extension used to conflict the tcpdf library. -

This solved the issues. I renamed the cjlib folders en after that the installation went smoothely. Now I can check what needs to be done after the migration of categories... -

You could try that. However I believe this issue is being caused because you are installing it on server based on Windows. So please check if the folders have right... -

Please try installing using directory installer method. Extract the package to a temporary location on your server and use that directory path to install. -

Looks to be the issue with PDF generation. Can you try to disable send pdf report option in your survey and check if this happens? -
