These are editor buttons. Go to Extensions -> Plugins -> Filter Editor - XTD plugins -> Disable them. -

So your menu option overriding your component option. Please set it to Use Global and your global option will work. -

It looks like the scripts were combined using your template's minifier option in wrong order. Can you please try to disable that option to combine and minify the scripts? -

The missing items could be trashed. The dashboard shows only published responses. Go to Responses page, filter state as "All" and publish them. -

I am not sure why they are denied, it may be because your Joomla was upgraded from 1.5 version and the tables were not upgraded properly. Other reason could be the migration...
