My guess is correct. For poll with id 26, you have votes recorded for options with ids 84,85,86 which is correct. But at the same time there are some votes with ids 144,145,146 and 146 for which no answers exist. Hence you see blank values. Delete these votes. -

That's what my confusion also. You have "CjBlog - Categories" menu item somewhere but is not shown in any of your menu items. Strange, may be a bug in Joomla. For now, what we can do is search that menu item in the database directly. Please send me the xxx_menus table data. Or you find row with the alias column value as "blog" and let me know the id of that row. -

I don't see any change by enabling or disabling cjblog content plugin. I enabled content plugin now and can see both the urls mentioned in your first post above and last one in this topic are working fine. No blank page shown. So you must have disabled some other plugin. Please check. -

Well, there is no way the component can add that url. It must be from some menu item, this is the reason why I am asking you to let me know where can I see that. I searched everywhere but could not find it. How did you get to know about such url? you must have created it sometime earlier. -

Not exactly. I would like to see the menu item associated with the url https://www.isknow.how/blog which is what not working for you. The menu item in your screenshot points to https://www.isknow.how/iskh-blog which is working fine. -

Looks like I missed something here. Are you referring to the images/files attached with upload type question while user responding to survey?or the images attached to the questions while editing survey? In the first case it is /media/communitysurveys/uploadsIn second case it is /media/communitysurveys/images -

Ok, let me explain. Your first url which is showing header: https://www.isknow.how/iskh-blog/iskh-blog-articles/latest This url is pointing to a menu item "alias" as iskh-blog. https://www.isknow.how/blog?view=categories This url is pointing to a menu item with "alias" as "blog" These are urls from two different menu items. But I can see you have assigned your header module to menu item "iskh-blog" but there is no "blog" menu item at all. Hence the header do not show in later url. Hope I explained clearly. -

Unfortunately I could not reproduce this issue with multiple ipad emulators. So I must check this only on ipad. Rather I chose to replace jQuery raty plugin with jQuery bar rating. Hopefully this will not have same issue because this plugin has native bootstrap based rating function. -
