The template will be same for every survey. When you go to invite page you can customize it and send invites. You can use the template as base version to customize. -

Yes, but the questions are selected from all quizzes in the selected category not the same quiz. -

Please disable CB integrations if you do not have Community Builder installed. You can disable them from Community Surveys Options. -

Are you using bootstrap3 layout? do you have the folder components/com_cjblog/layouts/bootstrap3? -

If your original images are small size, there is nothing I can do about this here. However if they are bigger, go to /media/com_cjblog/thumbnails/ and delete the files and retry. -

Edit components/com_cjblog/layouts/default(or bootstrap3)/articles_list.php and find $thumbSize = $params->get('list_thumbnail_size', 96); change it to $thumbSize = 48; -
