It is hidden under the css. Try adding below css override in your template css file #cj-wrapper .category-icon img {min-width: 24px;} -

Can you please update v5.2.1, you should be able to see the response report in this version. However the report is shown after completing the quiz, not after every question. -

Did you restrict access to the component or user group? There is another option in JCE profile that restrict the edit available only for backend. Please check them. -

The category quizzes are not regular quizzes. The questions are selected randomly, and the regular quiz flow is not available for them. -
