Your database table seems to be not updated properly. Can you please let me know the version of the component you are using? The sociable groups are not same as Joomla... -

Which editor are you using? You can change the editor from the component options. Try JCE editor which has image selector and other advanced features. -

Looks like you did not create any menu items, so it is using your homepage menu item id. This is the default behavior in Joomla. To show the surveys on their own... -
