This is the known issue. The Joomla category system do not fetch the nested level counts due to too many database calls it needs to fetch the data. -

You may be checking wrong file or you have a wrong plugin version. The plugin file is plugins/content/surveys/surveys.php and this file should contain 228 lines. -
