This is caused by the width of the attachments block which is not wrapping to next line. I will check for a workaround to fix it. -

For form page, the itemid is not being calculated and simply forward with request parameters. I will fix it with next release. -

Please disable Show Report option of your survey or disallow View Results permission. Then user will be redirected to the url. -
