The issue is because you set the survey as private. You can't link a private survey to a menu item, set it as public. -

There is no such error message in the plugin, it is generated by the CB component. Can you please let me know the steps to reproduce it? -

I just accessed the global survey url and I dont see any 403 error on clicking start survey button. I am able to see the survey. Did you fixed it already? -
