Certainly not, in fact there is no such default date format defined anywhere in CQ. By any chance do your quiz options has override for the pdf date format... -

The image tags are not saved at all. Please check the text filters in your global configuration. Also check if you can save images with any other component such as articles... -

CQ only formats the date as per pdf date format option in your component options. Actual date and timezone are used as it is used by Joomla, i.e. your server time. -

By default user's name is shown on the pdf. i.e. whatever the name entered in Joomla profile. Do you need to show first and last name from some other component? -

This is a known issue. Please edit components/com_communityquiz/helpers/tcpdf.php and remove below... -

It may be some issue with the path of the folder JCE is mapping the images to. Can you let me know a sample post where I can see it? -

The message is already shown when the user already voted. But the form is also shown when the user does not have access to results. I will add an option to hide them. -
