Your images are entered with a relative url. So when the page is loaded, the images will use the url relative to the current url. For example. -

You cannot change them using custom answer placeholder option. You can change them in the language file languages/en-GB/en-GB.com_communitysurveys.ini -

You need to disable debug mode in the component options. Go to Components -> Community Surveys -> click on Options button on toolbar -> Integration tab. -

1/2 - When you open a response, it will be moved to the respective survey reports page. So going back will show the reports page of that survey. Hence you see that listing....
