The list limit can be configured in your global configuration. btw.. there will be only one query fired to get all the 100 questions and not 100 queries. -

The issue was caused by the incorrect layout setting. Your template is based on Bootstrap 4 but you have selected default (bootstrap2) layout and hence the conflict with css.... -

Page cache doesn't go well with dynamic content. It is strongly recommended to turn of that plugin and use conservative caching option in global configuration. -
