I went to the above URL, click on Test menu and List menu. Both loads the surveys fine. Did you already fixed the issue? -

No. The system plugin causes issues by caching the pages and they do not render properly. Better not to enable it. -

You need to create the menu items for at least all TOP level categories. Otherwise they will default to home page menu item by Joomla. -
