There is no menu available for it. However, you can access Most Answered Questions and Unanswered Questions from the toolbar menu on front-end. -

What is the URL in the email sent to you? The URL in the editor may not be the actual URL. Send a test mail and see what is the result. -

That looks to be a bug. The update can be done from your Joomla updates page (Extensions -> Manage -> Update) or you can manually download and install the package. -

These fields can be used to add your custom css class name to the tags displayed on response page. If you would like to customize the css for a question, you can use them. -

Page header question has the class name "qtype-page_header". So you can target your styling to #cj-wrapper... -

That's the default setting of Joomla. All custom permissions will get default not allowed status.
