Removing modules for Template View

That means your template is loading modules in component view which is a bad design. Unfortunately, community surveys cannot do anything with such custom designs. If your template follows the Joomla template design, the component layout should not have any modules.

Do I need to create an menu item for it? If so which one do I use? 

Yes, menu items would help. Create a Survey Category menu item for each of your TOP level categories.
