What is the editor you have set in the CjForum options? Your editor buttons may create such problems, try to disable all Editor-XTD plugins and check. If you still see the... -

Are you publishing any other modules on the same page? Community Surveys don't use GuzzleHttp. Can you send me your website details to check and understand the issue?
