Try to change bootstrap version. Go to components->cjforum->click on options button on toolbar->change default layout to bootstrap3. -

You can enabled attach pdf option which will deliver the user responses in a pdf. -

You can add some info about the columns in the question description. Dual grid is not supported, use two grid questions instead. -

This error may come when emal could not be sent. Please check your email settings in Global Configuration. Are you receiving emails from any other component? -

It's a css conflict. Add this in your template css override file or custom style option if provided by your template. #cj-wrapper .media-body {width: 100%} -
